LIST OF REQUIREMENTS FOR VISA
Original old and new passports
Photocopy of passports (first page, last page and all pages with immigration stamps and visas)
Schengen visa form
Passport sized photos (35mm x 45mm, white background, both ears exposed, closed mouth, no eyeglasses, w/ collared shirt/blouse).
For spouses of Spanish/EU nationals โ Spanish/EU marriage certificate (issued not more than 6 months ago) recognized by the Government of the corresponding EU national. Marriages that are not registered/recognized by the corresponding EU Government will not be processed as an EU spouse visa. Only marriages recognized/registered by the corresponding EU Government are free of charge.

Copy of roundtrip flight booking
Hotel vouchers for the entire stay in Schengen area.
Official invitation letter issued by Spanish Police and photocopy of the sponsorยดs passport and/or D.N.I., residence card, etc.
Certificate of employment and leave of absence approved by employer (if employed)
If self-employed (copy of business permit, SEC registration, DTI registration)
Copy of recent Income Tax Return
For nonโFilipino applicants, please present your ACR, ICR and reโentry permit (and 1 set of photocopies)
Bank certificate under applicantโs name attached with copy of statements (3 months)
Copy of credit card statements, land titles and other economic means.
Travel Health Insurance covering the entire period of the personโs intended stay and valid for all Schengen states with a minimum coverage of EUR 30,000
Additional documents for minors
-If the minor is not travelling with either parent, original and copy of DSWD travel clearance.
-Affidavit of support and consent executed by both parents
-School certificate and leave of absence if travelling while classes are ongoing

Bank Certificate and statement
