- a declaration signed by the person who will be responsible for you in New Zealand
Current and old passports (signed on the 3rd page for new passports)
With colored photocopies of first page and all pages with visas and immigration stamps as well the last 2 pages of passport • 2 passport size photos
Signed consent letter • Employment certificate (if employed)
Copy of registration, DTI, SEC (if self-employed)
Copy of recent ITR • Recent Bank Certificate, addressed to New Zealand Embassy
Bank statements for the last 3 months • For student, original School certificate • For minors, affidavit of support and consent to travel from both parents
NSO Birth certificate
NSO Marriage contract (if married)
Copy of hotel booking in New Zealand
Copy of roundtrip flight reservation
