REQUIREMENTS
LIST OF REQUIREMENTS FOR VISA

Completed Schengen visa form (for minors, both parents should sign the application form)
Two (2) passport size photos (white background, no eyeglasses, w/ collared shirt/blouse, both ears exposed, neutral expression)
Current and old passport
Photocopy of visas and stamps
If employed, please submit Certificate of Employment detailing the monthly salary, hiring date, company contact number and address.
Approved leave of absence and copy of pay slips for the last 6 months
Copy of latest ITR
Copy of PRC license (if applicable)
Copy of Alien Certificate of Registration (ACR), re-entry permit for -Filipino applicants
For government employee, submit copy of travel authority
If self-employed, please submit copy of business documents such as SEC registration, BIR registration, DTI, permits, Ask for Receipt signage and latest Income Tax Returns
Original Bank Certificate under applicant’s name with corresponding 6 months statement of the account(s) declared thereof (1 year bank statements for first time in Schengen
If available, copy of proof of assets such as but not limited to; land title(s), investments, bonds, or other economic means
For students, please submit Certificate of enrolment, copy of ID and leave of absence if travelling in term time
PSA Birth Certificate
PSA Marriage Certificate, if applicable
Copy of roundtrip flight booking • Detailed day-to-day itinerary
Copy of hotel bookings/vouchers for the entire stay in Schengen
Travel Health Insurance covering the entire period of the person’s intended stay and with a minimum coverage should be EUR30,000 (with additional 15 allowance beyond the travel)

Additional Documents for minors:

If the minor is not travelling with either parent, original and copy of DSWD travel clearance.
Affidavit of support and consent to travel executed by both parents
For Incentive trips/business trip sponsored by the company, submit the following:
Original Letter of Guarantee from the company
Copy of business documents such as SEC registration, permits and latest Income Tax Returns
Company Bank Certificate
If Invited by a company in Italy
Invitation letter from the host company (Lettera di Invito per Affari) accompanied by proof of identity of the signatory
Copy of Camera di Commercio of the inviting company
Proof of transactions of local company with the host company
If invited by a relative or friend in Italy
Invitation letter/Dichiarazione Garanzia E/O Allogio
Proof of identity (passport and residence permit of inviter)
Proof of relationship
