LIST OF REQUIREMENTS FOR VISA
Completed application forms -Form 1419
-Form 956
-Subclass 600 form
-Consent form.
Copy of new and old passports (first page and all pages with visas and stamps)
Copy of Alien Certificate of Registration (ACR) for non-Filipino applicants
2 passport size photos (white background, no eyeglasses, both ears exposed, neutral expression)
If employed: Certificate of Employment and copy of ITR 2316
If self-employed: proof of business registration such as SEC registration, DTI, business permits and Income tax returns
Bank Certificate (addressed to Australian Embassy Manila) and 3 months bank statements
Proof of other financial means such as copy of credit card statement, investments and land titles
For students: evidence of enrolment such as school certificate and school registration:
For children travelling with one parent:
1. Duly accomplished Form 1229 by non-travelling parent, and parentโs contract information and valid government-issued ID with signature
For children travelling with someone other than a parent:
1. Duly accomplished Form 1229 by both parentsโ and both parentโs contact information and valid government-issued ID with signature; and,
2. Duly accomplished Form 1257 by the person who will be responsible for you in Australia and valid government ID
Copy of birth certificate and marriage contract if married
Copy of itinerary
If visiting a relatives or friends, provide all documents below:
1. Letter of invitation from your relative or friend in Australia
2. Copy of passport biopage
3. Evidence of your relationship to your relative or friend
4. Evidence that they have the necessary funds to support your intended stay
